Alcantara underwent Tommy John surgery Friday to repair the UCL in his right elbow, Craig Mish of the Miami Herald reports. Alcantara landed on the injured list in early September and had his rehab assignment cut short after spraining his UCL on Sept. 22. Marlins' Sandy Alcantara: Shut down with UCL sprain
Alcantara (elbow/forearm) acknowledged Saturday that he will not pitch again this year, Jordan McPherson and Andre Fernandez of the Miami Herald report. Alcantara threw four scoreless innings in a promising rehab start Thursday with Triple-A Jacksonville, but his elbow and forearm were barking again in the aftermath and he's now officially been shut down with what's being called a right UCL sprain. The reigning NL Cy Young Award winner said it has yet to be determined whether the injury will require a surgical repair. Marlins' Sandy Alcantara: Renewed forearm tightness
Alcantara felt renewed forearm tightness following his rehab start Thursday with Triple-A Jacksonville, Jordan McPherson of the Miami Herald reports. Alcantara is headed to Miami to be evaluated this weekend by the Marlins' medical staff, and it's a safe bet that he is not going to be able to return before the end of the regular season. Marlins' Sandy Alcantara: Fares well in rehab outing
Alcantara (forearm) allowed one hit and no walks while striking out four across four scoreless innings in a rehab outing with Triple-A Jacksonville on Thursday, Jordan McPherson of the Miami Herald reports. Alcantara put up an impressive stat line, but more importantly he managed to throw 51 pitches while mixing in all of his offerings. He also topped out at 98.3 mph. Alcantara (forearm) is scheduled to make a rehab start Thursday with Triple-A Jacksonville, Craig Mish of the Miami Herald reports. If that outing goes smoothly, Alcantara could make it back for one final regular-season start and perhaps help the Marlins in their quest to claim an NL wild-card spot. It was his first time throwing a baseball since he landed on the 15-day injured list last Wednesday with what has since been diagnosed as a right UCL sprain. Alcantara held Tampa Bay to just one run through five innings, but the floodgates opened in the sixth when three hits and a hit-by-pitch resulted in three runs and Alcantara's removal from the game. Alcantara (6-11) took the loss Wednesday, giving up four runs on seven hits and three walks over 6.2 innings as the Marlins fell 4-0 to the Padres. He struck out three. Last year's NL Cy Young Award winner didn't have his best stuff, managing only eight swinging strikes among his 105 pitches, but Alcantara was also backed by an offense that only managed four hits off San Diego pitching. Alcantara (6-10) allowed three runs on seven hits and a walk while striking out six over six innings to earn the win Friday over the Dodgers. Alcantara was tagged for three home runs, but they were all solo shots, and the Marlins responded with five homers of their own. Alcantara has gone at least six innings in eight straight starts, posting six quality starts and a 3-3 record in that span. Alcantara had allowed just one run over his previous two starts combined, but the Rangers were able to tag him for a season-high three home runs. Alcantara did not factor into the decision Tuesday, allowing four hits and a walk over eight scoreless innings against Philadelphia. He struck out five. Alcantara is finally starting to look like he did in his 2022 Cy Young campaign. He's now thrown 17 innings over his last two starts, allowing just one run on nine hits in that span. However, Alcantara was stuck with a no-decision Tuesday as the Phillies rallied to score three against David Robertson in the ninth inning. The 27-year-old Alcantara's pitched to a 2.02 ERA over his last five outings, lowering his ERA to 4.21 on the season with a 1.21 WHIP and 118:38 K:BB across 22 starts (143.1 innings). Alcantara (3-9) took the loss Wednesday, allowing four runs on eight hits and two walks over six innings against the Cardinals. He struck out seven. The Cardinals tagged Alcantara for four runs in the first inning, highlighted by a Nolan Gorman three-run homer. Alcantara was able to settle down and hold St. Louis off the board over his final five frames, but Miami couldn't overcome the early deficit. The 27-year-old right-hander had shown some improvement recently, delivering quality starts in three of his previous four outings. Still, it's been a disappointing campaign overall for Alcantara. Alcantara (3-8) took the loss Friday, giving up three runs (two earned) on eight hits and a walk over six innings as the Marlins were downed 5-2 by the Orioles. He struck out five. For the first time this season, Alcantara delivered back-to-back quality starts. but Miami couldn't provide him with enough run support to get the right-hander into the win column. The team continues to keep a close eye on Alcantara's workload -- it's been nearly two months since he tossed 100 or more pitches in a start, but that hasn't resulted in significantly better numbers, as he has a 4.26 ERA, 1.29 WHIP and 45:17 K:BB in 63.1 innings in those 10 outings. Alcantara allowed at least one home run for a second straight game and the 10th time over 18 appearances this season. However, that was the only damage done against the right-hander Friday, and he was in line for the win until A.J. Puk blew a two-run lead in the top of the ninth. Alcantara lasted just five frames Sunday, his shortest start since May 2. He surrendered multiple homers for the first time all season, as Orlando Arcia tagged him for a solo home run in the second inning and Ozzie Albies swatted a two-run shot in the fifth. The outing continued a rough patch for Alcantara, marking the fifth time he's allowed at least four runs across his past seven appearances.
